HOW DOES THE HOSPITAL PLAN FOR DISCHARGE?

PATIENT PRESENTS READY FOR DISCHARGE HOME

Please note that the hospital Discharge plan will have been constantly monitored and reviewed since admission.

Once ready for home the following considerations will have been made via assessment by the Home Discharge/Brain Injury Teams:-

Social (to include family circumstances), Welfare, Physical, Cognitive ability, Functional Independence.

At this time a plan may be put in place for continued input from specialist care team.

PATIENT PRESENTS WITH CONTINUING HEALTH CARE NEEDS

Please note that the hospital Discharge plan will have been constantly monitored and reviewed since admission.

Patient may be fit for discharge from hospital however may not ready to go home.

It may be necessary for the patient to access further brain rehabilitation.

This may mean that the patient is referred on to a specialist unit which may be out of the county of residence.  This means that the patient will be away from the family home for a period of time. However, Northamptonshire has many specialist services available.
